Quick health check

Start by selecting the condition for which you require a treatment by using the conditions menu at the top of the page, or by selecting the relevant condition on the homepage. Once you have chosen your intended treatment, click the “Start Consultation” button, which will take you to a medical assessment questionnaire.

The medical assessment questionnaire is secure and confidential and usually takes a few minutes to complete. The medical questionnaire will ask you about the treatment you are seeking, the nature of your specific condition and your medical history. These short questions are similar to the questions your GP would ask you in a face-to-face consultation.

Once complete, your medical assessment will be reviewed by one of our independent prescribers who will assess if the treatment option is suitable for you.

If you have any questions when completing the medical assessment or when selecting the treatment options, please call our patient support team on 0207 867 3932 where one of our pharmacist will be available to help.
